Laura Wagner is a principal investigator on a new 18-month grant from the Rita and Alex Hillman Foundation to help UCSF Street Nursing improve care transitions by partnering with the UCSF Emergency Department Navigation Team’s ED Information Exchange (EDIE) program. Also partnering is Lava MaeX, an organization that provides mobile hygiene services two days a week to the houseless population of San Francisco and Oakland.
